A Study of Activities of Daily Living and Disability Level in Elderly with Physical Disabilities

Abstract
Objective : The purpose of this study was to identify the differences in the ability of activities of daily living(ADL) according to the disability level in elderly with physical disabilities and to clarify the relationship between disability grade and sub-domain of ADL.

Method : Current investigation used raw data from 12th Korea welfare panel study. Among the total of 15,422 subjects, 495 elderly with physical disabilities were included in the analysis. General characteristics were set as age, gender, income, educational level, and disability grade. The ability of ADL was evaluated as 12 sub-domain of basic activities of daily living(B-ADL) and 10 sub-domain of instrumental activities of daily living(I-ADL). In the main analysis, the researchers performed Spearmann correlation analysis and binomial logistic regression analysis.

Result : In the correlation analysis, disability grade showed the highest correlation with bathing of B-ADL(r=-.315, p<.01) and grooming of I-ADL(r =-.302, p<.01). In the B-ADL total score, the probability of 'minimal assist' was significantly higher in the severe disability group compared to the mild disability group (OR=4.241, 95% CI 1.950-9.224, p<.001) in reference 'independence'. In the I-ADL total score, the probability of 'minimal assist'(OR=2.513, 95% CI 1.311-4.817, p=.005) and ¡®maximal assist¡¯(OR=4.837, 95% CI 1.229-19.030, p=.024) was significantly higher in the severe disability group compared to the mild disability group in reference 'independence'.

Conclusion : The higher the disability grade of elderly with physical disabilities, the more help was needed for bathing of B-ADL and grooming of I-ADL. The higher the disability grade, the more help was needed for the all sub-domain of ADL. Therefore, it is necessary to develop a community occupational therapy program to improve ADL ability of elderly with physical disabilities according to disability grade.
